Job Description
Wellness Nurse Coordinator – part-time
Function: Responsible for planning, coordinating and implementing the health and wellness programs for seniors
Reports to: Social Service Administrator
Hours: Flexible part-time work schedule, 15 to 20 hours per week.
Pay range: $40-45 an hour
Duties and Responsibilities:
 Coordinates health screenings and consultation services for older adults; provides health counseling; maintains records of results/recommendations.
 Keeps program records, and ensures compliance with local, state and federal regulations.
 Collaborates to assess home health needs of the agency’s clients in the home and develops appropriate     action plan.
 Plans, facilitates, and researches topics for support groups. Conducts related meetings.
 Provides direct services within the scope of practice and skill level.
 Conducts Quarterly education seminars/training per SDS (Senior Day Services) licensing requirements.
 Initiates and follows up on referrals for wellness related services.
 Serves as substitute to the Social Services Administrator for the SDS program.
 Monitors AED equipment, First Aid boxes, and sharps containers.
 Attends Yu-Ai Kai trainings.
 Attends to any other duties needed for the smooth operation of the agency.
Qualifications:
 Associate’s or Bachelor’s Degree in Nursing with a current California R.N. License required.
 CPR/First Aid certified.
 DOJ (Dept. of Justice) and FBI Fingerprint clearance.
 Complete Criminal Record Statement per Department of Social Services.
 Must have a health screening and TB test that is not more than a year old from date of hire and no later than seven days after employment.
 Fully vaccinated and boosted against COVID-19 or willing to become fully vaccinated against COVID-19 prior to employment and boosted when eligible.
 Valid CA driver’s license & proof of insurance.
 Must be able to maintain confidentiality.
 Positive interpersonal skills.
 Excellent organizing and prioritizing skills.
 Ability to work in a fast paced environment with frequent interruptions with minimal supervision.
 Experience working with the older adults preferred.
 Bilingual (Japanese/English) and Bicultural understanding a plus.
 Knowledge of and interest in the API/Japanese/JA community and culture, and its elders.
Benefits:
 Flexible work schedule.
o Set up a schedule that is 15-20 hours a week between 8 AM and 5 PM.

o Must agree to work 20 hours/week to qualify for Dental benefits.
